APPLICATION INSTRUCTIONS FOR ENROLLMENT IN MA-CHIS INDIAN TRIBE OF ALABAMA

Files on each member of the family are necessary. Proof must accompany each application.
 
PRINT the information in the blanks of the forms in black ink. Please put each application in a folder with the name on the front of the folder.
 
Fill out a genealogy chart showing your lineage back to your Creek Native American ancestor. This will be filed with your application.
 
Attach photocopies for birth certificates, marriage licenses, divorce certificates and death records.
 
Each photocopy page must have the name of the applicant at the top.
 
Under the item on photocopies state the following:
 
A. Name of person document refers to.
 
B. Generation-(lst, 2nd, 3rd, etc) This allows for more than one document to be photocopied on one page.
 
C. Obituary or newspaper items used must be labeled with:
 
1. Name of paper
 
2. Where paper was distributed
 
3. Date of issue that item appeared in.
 
KEEP A COPY OF ALL ITEMS FOR YOUR PERSONAL FILES. This will act as a backup in event of disaster or lost of records. Standards of proof are needed for each person that includes data concerning:

(1) birth, date and location,
(2) marriage, date, location to whom
(3) death, date, where died, where buried.
 
*The tribe requires a notarized statement accompanying the documents to testify and swear that all proofs and information are true.
 
The following contains acceptable items of proof of each generation.

CENSUS RECORDS (Be sure all are labeled with date of census, town and state of census),MILITARY RECORDS, FAMILY BIBLES (photocopy pages include the title pate and copyright page.Label whose Bible it was, who has it now and where it's presently located), SCHOOL OR CHURCH RECORDS, LAND RECORDS, LAND GRANTS, BIRTH CERTIFICATES, NEWSPAPER OBITUARIES, PENSION RECORDS, TOMBSTONE, YEARBOOKS, UNDERTAKER'S OR MORTUARY RECORDS, HOSPITAL RECORDS, PERSONAL BIOGRAPHIES, OLD INSURANCE RECORDS, PERSONAL ARTIFACTS (grandfather's old bills, etc.), OLD WAR LETTERS. You may have something that is not listed here.
